Faculty & Staff ID Card Issuance and Renewal/服務證核發及換發

New Full-time Personnel

  • Eligibility: New full-time faculty, staff, technicians, security personnel, and full-time contract employees.
  • Process: During the check-in process, please upload a digital headshot (JPG format) via the Electronic Form – Personnel Processing Form.
  • Timeline: Cards are produced within three weeks after the reporting date and once all required documents are submitted.
  • Collection: Once the card is completed, it will be delivered directly to your affiliated department or office.

Adjunct Faculty

  • Part-time faculty members who require an ID card should fill out the attached "National Tsing Hua University Faculty & Staff ID Card Application Form" and email it along with a digital photo to the designated mailbox.

How to Apply for Replacement/Reissuance

If you need a new card due to loss, damage, or other reasons, please apply via the following system:

  • Path: Academic Information System →Other Services in Campus → ID Card(Electronic ticket) Loss/Reissue/Password Changing System.
  • Instructions: Detailed system operation procedures are provided in the attachment.
  • Fee: A replacement fee of NT$200 will be charged for reissuing the card.

Collection Method

  • When the replacement card is ready, you will be notified via email.
  • Upon receiving the notification, please come to the Personnel Office to collect your new ID card.
