If you want to apply resignation or your contract is expired, you can start the process in Academic information system 10 days before your last date in school. (If you can’t start the process, you need to shorten your contract period in system first and get the approval from the Personnel office.)
You most provide this form (not copy one) before your last date in school. If you don’t follow the rule to cancel the insurance, we will charge you extra insurance fee (include employers ‘ and personal insurance fees ) and your resignation process will also be affected.
